Disclaimer

Our desire is to help the military personnel retain ownership of their animals, have these pets cared for in a loving manner while their owners are absent,and have these animals returned to their owners once their owners return.

If said pet dies or becomes accidentally injured, ill, or lost; Owner does not hold the Agency and Caregiver responsible and releases the Agency and Caregiver from any claim arising from said injury.

If said pet bites or injures any human or other animal, Owner will hold harmless and indemnify, and protect Agency and Caregiver, from any claim or suit filed by anyone as a result of such an incident.

Liability Wiaver

In signing this application, I understand and agree to the following:

I understand that all of Paws for Deployment records are to be kept confidential.

I relieve Paws for Deployment of any liability and accept the responsibility to pay any and all medical bills connected with any injury, real or alleged, incurred while I am engaged in any duties as a volunteer.

If I am unable, I authorize the Paws for Deployment to seek emergency medical treatment in case of accident, injury or illness.

I, _______________________________________, hereby agree to Indemnify and hold harmless the Paws for Deployment, its employees, agents, officers and directors, from any and all liability arising out of or in consequence of, or injury to myself or others sustained as a result of, any activity connected with volunteering for Pets for Deployment.

I attest that I understand the important issues addressed in the above rules.

By signing, I agree to adhere to the conditions and requirements of this agreement.

I affirm that I have never been charged with animal cruelty.
